Background:
Patient safety is high on the agenda and especially when in situations such as under the knife one wonders whether s/he is in safe hands.

Aim of the symposium:
This one day conference aims to:
- raise awareness of the current patient safety issues within the operating team
- identify the reasons why errors occur
- recognise how these errors can be reduced.

Learning objectives:
By the end of the conference delegates will
- understand how and why errors occur within the surgical team
- understand the importance of human factors in reducing errors within the surgical team
- recognise key strategies which help to avoid errors within the surgical team
- provide the theatre team with an understanding of the cause and effect of surgical error and provide better recognition of the effects of human factors on performance.
